• When a dock worker contracts any disease specified in Schedule IV, a notice in Form XIV shall be sent by the employer forthwith to the Inspector.
• If any medical practitioner attends on a dock worker suffering from any disease specified in the aforesaid Schedule, the medical practitioner shall without delay send addressed to the Director General, Directorate General Factory Advice Service and Labour Institutes, Central Labour Institute Building, Sion, Bombay- 400022, a notice stating the name and full particulars of the patient and the disease.
